We prepare an annual memo to directors and executive officers that asks them to report all related interests. The instructins are cut and paste definitions directly from the regulation and are not particularly helpful. I would like to revise this format into more of a Q&A flow chart. Does anyone have a system that works that they would be willing to share?

In that thread a member offered to share their questionnaire/survey with another member and may be willing to share with you as well.
If you aren’t able to obtain a sample, please post in this thread again letting us know.
