We are looking to implement an authorization form which would allow use credit reports to cross-market products to members as well as to get consent for text messaging for advertising purposes. I have been unable to find a lot of guidance on this form and was wondering if anyone else had something of that nature they have in place at their shop.

Or if no one does, maybe point me in the right direction of what I should be aware of when creating the document. Thanks.

You will need to consider restrictions and requirements under Fair Credit Reporting Act and Telephone Consumer Protection Act.
